The formula:
1. The first 3 digits are always 992
2. The fourth digit is either a 0 for new patients, a 1 for established patients, and a 4 for referred patients. The 0 or 4 can only be used once!
3. The last digit is 1-5. Let me make life easy, do not ever bill a level 5 unless you are 100% sure. The last digit decision use is based on time and complexity of exam.
