Patient Demographics

  • You only need to enter a Patient’s information one-time to have them saved to your Patient List. You can edit their information if you need to make corrections/updates to their Personal information or their Insurance information. Please avoid entering a Patient more than one-time and the next training section “Benefits & Eligibility Request” will talk show how to add a Secondary Medical policy when there is a spouse or a second parent with an active Medical plan.
  • The Patient information (Home Address, DOB and Ph#) goes in top section. If the Patient is the Medical policy Subscriber/Holder, you can click the gray box next to “Copy Above Information” and all information will drop-down to save you from re-typing the same Information.
  • You will Only be entering Medical Insurance information for you Patients, never submit a Dental Insurance ID#. Only use an SSN# if the Patient’s Medical card says to use the SSN#.
  • Always include all Letters at the beginning of an ID# and not all Medical Insurance cards will have a Group# listed, you may leave that box blank.
  • If a Patient gives you a Military ID card for Tricare military medical plans, the “Benefits Number” will be on the back of the card, do not use the “DOD ID Number”.
  • If your Patient has a Blue Cross/Blue Shield plan from Outside of your State, you will only be able to view/choose the Blue Cross/Blue Shield plan of your State, you will not see an BCBS/Anthem plan outside of your State. The CODE team has programmed the Portal system to only let you choose your State’s Blue Cross/Blue Shield plan, because if a Claim is created, we have to send it to your State’s BCBS plan and they will forward it to the Patient’s Blue Cross/Blue Shield plan for us.
